GENERAL INSTRUCTIONS - Most of the documents on this page are fillable PDF forms. If the instructions in the form request that the form be returned by email, then save the form to your computer before completing. Once on your computer, the forms can be completed using Acrobat Reader or Writer and submitted by attachment through email. ATTENTION MAC USERS: When using Preview to complete the form, please save the file after entering your response using File > Print > PDF > Save As PDF. IF you use the default save settings, your entries will be invisible to Acrobat on a PC.
Faculty Admissions Forms
- General Application to Memorial University - For students who are new to Memorial University. Specifically, 1) students who have not attended Memorial University within the last two years, 2) recent high school graduates, and 3) transfers students from another post-secondary institution.
- Application for Engineering One or Academic Term 3 - For current MUN non-engineering students who are wishing to switch their area of study to engineering OR current MUN engineering students who wish to switch their major
- Application for Re-Admission to Academic Terms 3 through 8 - For students seeking to rejoin the Faculty who were previously admitted to Academic Term 3 through Academic Term 8
- Admission Acceptance Card
- Course Change Form - This form is for the purpose of making changes to your registration and should be used when such changes cannot be made through Student Web or the Telephone Registration System.
- Deferred Examination/Work Term Report/Presentation Form
- Course Load Waiver - For students who wish to register for more than 6 courses.
- Prerequisite Waiver Request - For students who believe they have equivalent course credit for a pre-requisite course.
- Course Exemption Request - For students who believe they have equivalent course credit for a required course.
- Engineering One Course Waiver Request - For students who believe they have equivalent course credit for a required Engineering One course.
